Current Research

Stroke and TIA (mini-stroke)

Prevention

Aspirin and Plavix for Stroke Prevention
Response study: The study is being conducted to find out the effect omega-3 fatty acids have on subject responsiveness to dual-antiplatelet therapy after neuroendovascular stenting and on subject receiving dual antiplatelet therapy for stroke prevention.

Generalizable Knowledge

Impact of Emergency Department Discharge on TIA Rapid Assessment Outcomes: A Prospective Observational 90-Day Follow-Up Study (TIARA)
Veznedaroglu (PI)
This study measures outcomes following TIA to determine the impact standard practices (admission or discharge) on stroke and TIA recurrence.

Long Term Outcomes of Octogenarians with Acute Ischemic Stroke who Receive Endovascular Therapy
Veznedaroglu (PI)
This study will examine the long term outcomes in patients 80 years and above who have had an acute ischemic stroke and have received IV tPA, Heparin, and/or endovascular treatment.

Prehospital Stroke Alert (PHSA)
The primary aim of this study is to determine a decreased time from when the patient arrives to the hospital to when they receive IV tPA by delivering patients directly to CT for imaging, by passing ED triage. Published

Improved Door-to-Needle Times and Neurologic Outcomes When IV Tissue Plasminogen Activator Is Administered by Emergency Physicians with Advanced Neuroscience Training
This study demonstrated improved door to needle times and outcomes in Capital Health specialized neurological emergency department.

Retrospective Study of Heparin Administration for Ischemic Stroke When There Is an IV-tPA Contraindication
Veznedaroglu (PI)
This study will analyze outcomes of patients that receive IV Heparin within 24 hours of symptom onset. The aims of the study are to compare early bleeding complications between patients administered heparin when they presented with contraindication to IV tPA but within 24 hours of ischemic stroke onset and also patient outcomes. Published

CLOT Retrieval
Study looking at the clinical significance of retrieving actual clot vs. not when performing thrombectomy for stroke. We would like to start prospectively collecting data and we will document during the procedure if we visualized a retrieved clot or not then will test the clinical significance. Endpoint Outcome: TICI 2b or better if clot retrieved or not.

Trevo-pro-retreiver in Acute Ischemic Stroke Patients
Veznedaroglu (PI)
Post-market single center analysis. Published

Stroke Incidence with CAS
Veznedaroglu (Co-PI)
A retrospective review of stroke incidence following carotid angioplasty and stenting in carotid stenosis patients.

Emergent Treatment

DAWN–Trial - DWI or CTP Assessment with Clinical Mismatch in the Triage of Wake-up and Late Presenting Strokes Undergoing Neuro-intervention
Veznedaroglu (PI)
To evaluate the hypothesis that Trevo thrombectomy plus medical management leads to superior clinical outcomes at 90 days as compared to medical management alone in appropriately selected subjects experiencing an acute ischemic stroke when treatment is initiated within 6-24 hours after last seen well.

POSITIVE - Perfuson Imaging Selection of Ischemic Stroke Patients for Endovascular Therapy
Veznedaroglu (PI)
The purpose of this randomized trial is to demonstrate the safety and efficacy of mechanical thrombectomy over medical therapy (MT) for treating acute ischemic stroke patients ineligible for IV-tPA with persistent symptoms within a 12 hour time window from symptom onset as selected by physiologic perfusion imaging criteria.

TREVO Registry
Veznedaroglu (Multi-site PI)
Prospective post – market registry of international sites for TREVO device for thrombectomy

WEAVE Wingspan Stent System Post Market Surveillance Study
Veznedaroglu (PI)
The purpose of this study is to offer the use of Wingspan Stent and Gateway Balloon Catheter devices for treatment of patients with neurovascular disease, including recurrent stroke refractive to medical therapy.

Multicenter Outcomes after Treatment of Unruptured AVMs (ARUBA AVM)
Veznedaroglu (PI)
Review of our data and previous published literature shows that the rate of hemorrhagic strokes treated in ARUBA is significantly higher than previously reported. Our goal in this study to publish our thoughts on the fallacies of ARUBA trial and support it with data of results after interventional from similar cohort of treated unruptured AVMs.

Retrospective Recording and Analysis of Patient Outcomes Following Cyberknife Treatment in Arterio-Venous Malformations (AVM) in the Brain
This study will evaluate AVM treatment protocols and outcomes to ascertain if CyberKnife provides at least the same or better morbidity and mortality rates.

Generalizable Knowledge

Intracranial Aneurysm Treatment and Outcome by Dual-trained Neurosurgeons at a Single Center
Veznedaroglu (PI)
This is a retrospective review of both ruptured and unruptured aneurysm treatments and outcomes by dual trained neurosurgeons at Capital Health System, Inc. hospitals since commencement of neurosurgery. The hypothesis proposes to validate that aneurysm treatment for this patient population has evolved over time and new trends have developed in treatment paradigms.

Neuro-Oncology

Intra-arterial Avastin
Veznedaroglu (PI/IND holder)
High grade Gliomas in patients with gliobastoma multiforme, we will administer the chemotherapy, avastin, directly into the brain tumor via intra-arterial intracranial delivery. Delivery of chemotherapy directly to the brain tumor is a cutting edge method that may spare the patient the systemic side effects of the chemotherapy and allow for a more concentrated dose of the drug.
