Injury and Near Miss Reporting
Injury
EHRS requires employees to report injuries and near-misses in the workplace. All employee work-related accidents, injuries, and illnesses are assessed in a manner that identifies proximate and/or root causes which then may be used to develop specific corrective actions for the prevention of futures occurrences.
The injured employee is required to report the incident immediately. This form must be submitted within 48 hours after the incident. If the injured employee is unable to complete the form within this time frame, his/her supervisor should obtain as much information about the accident as possible and submit the required form(s).
Employees must complete an employee injury form and submit to the Office of Risk Management. Students must report injuries to their immediate supervisor. The student should complete the student injury form and submit it to EHRS.
In addition to the injury form, research personnel must complete the incident report form in Drexel’s Compliance Platform. The form can be accessed under the Forms link on the profile page. Click submit new form to access the form.
Near-miss Reporting
Near misses are accidents that did not result in an injury. Reporting near misses assists in creating a safe work environment for students, employees and visitors.
Typical near-miss accidents are:
- Minor chemical or biological spills.
- Exposed wiring.
- Slip, trip and fall that did not result in an injury.
- Sharp edges.
- Broken glass.
- Damaged doors.
- Water on the floor.
Report all near misses to EHRS. Provide your name, contact number or email, and description of situation. EHRS will initiate an assessment to determine the best action steps.
