Submit queries via a Data Warehouse Report Request Form (login only, click "Add New Item" to open the form for completion). Investigators may request one-time reports or set up a predetermined delivery schedule (e.g., daily, weekly, monthly, etc).

All appropriate Human Research Protection Program (HRPP) approvals must be in place before investigators are permitted to mine the College of Medicine's electronic medical record for research purposes. No reports will be provided without submitting evidence of HRPP and/or Privacy Board approval and a copy of the approved data collection instrument(s), where appropriate.

For projects involving PHI, data will be delivered in REDCap only. Users must gain REDCap access and create a REDCap database before any data is provided. To gain access to REDCap, visit the REDCap Portal on the DUCOM Research SharePoint site.
