Social Work at the Stephen and Sandra Sheller 11th Street Family Health Services Center
11th Street's Social Work department includes a master’s level social worker, an outreach and insurance enrollment specialist, a community health worker and an Americorp member. This team supports patients with barriers and life situations that can affect health and wellness, such as, applications for Medicaid, Medicare enrollment, Marketplace (or Obamacare) plans, prescription assistance, food stamps and other public assistance benefits, referrals for legal issues such as living wills, estate planning, SSI/Disability and landlord-tenant issues though the Drexel Legal Clinic and the Legal Clinic for the Disabled, as well as screenings and referrals for other issues like food insecurity, housing, transportation to medical appointments, social support and day programs for seniors, budgeting and setting self-care goals.
