Applicability

This policy applies to all Covered Entities within Drexel University.

Covered entities are defined in the HIPAA rules as (1) health plans, (2) health care clearinghouses, and (3) health care providers who electronically transmit any health information in connection with transactions for which HHS has adopted standards.

I. Policy

Drexel University (DU) affirms that the medical record is a record of patient care information that is objective, factual and chronologically developed to document intradisciplinary and multidisciplinary planning and intervention, to facilitate communication between members of the health care team, and to serve as a basis for review, study and evaluation of care rendered.

The patient has a right to request an amendment to information thought to be inaccurate.

II. Purpose

The policy provides the opportunity for the patient to correct inaccurate individually identifiable health information.

III. Procedure

  1. A patient or an authorized surrogate decision maker can request changes to the medical record in writing by filing a completed "Request To Amend Medical documentation" form found in this policy.
  2. The treating clinician and department administrator will consider the request and approve or deny in writing within 15 days.
  3. If the request is made in our role as a Business Associate, the primary Business Associate contact, if not the treating clinician, will authorize the request.
  4. Copies of all documents will be made part of the Medical Record.

IV. Standards

The treating clinician will review the request for changes to inaccurate information and review the medical record for accuracy. Medical Record process changes will be implemented to avoid repeat errors.
