Student Forms

  • Student Co-op Registration Agreement [DOC] – The Student Co-op Registration Agreement form is used to register placements obtained either through a self-directed job search or during the C Round.
  • Student Co-op Registration Agreement - International Students [PDF] – All international students are required to complete these forms and to submit a copy to both their co-op advisor and the International Students and Scholars services office.
  • Adding Classes During Co-op Form [PDF]  – A student who wishes to add a course while participating in their co-op experience, but has missed the week 9 time ticket for registration can do so using this form. The student is required to obtain the signature and approval of their co-op advisor for on-campus courses with a start time of 4:30 p.m. or earlier, prior to meeting with their academic advisor for registration. A co-op advisor will not approve an on-campus course with a start time of 4:30 p.m. or earlier without employer approval – use the Course on Co-op Employer Approval Form below to obtain employer approval for these courses. Evening, weekend, or online courses do not require co-op advisor signature on this form and can be provided directly to academic advisor for registration.
  • Course on Co-op Employer Approval Form [PDF] – A student who has registered for a face-to-face course with a start time of 4:30 p.m. or earlier during a co-op term is required to receive written approval from their supervisor at their place of employment to ensure it does not conflict with their work schedule. Please use this form to facilitate a conversation with your supervisor regarding the course you intend to take while on co-op. If approved, please fill out this form with the course information, receive your supervisor’s signature, and return to your co-op advisor by the deadline. Please inquire with your co-op advisor for the approval deadline for the term you are registering for. This form is not required for evening (start time of 5 p.m. or later), weekend, or online courses.

Employer Forms

Employer forms are used to register placements with new co-op employers, typically found through a self-directed job search.  This form must be signed by your co-op employer/supervisor. The original must be submitted with the Student Co-op Registration Agreement form (above); please ensure your supervisor/employer has a copy for their records as well.
