Partnership Comprehensive Care HIV Case Management

HIV Case Management at the Partnership

Clients who receive primary medical care for HIV/AIDS from the Partnership Comprehensive Care Practice (the Partnership) are eligible to receive medical case management services. Medical case managers at the Partnership are available to help clients get what they need to live with HIV. An HIV medical case manager will keep a patient's HIV status private, and only disclose HIV-related information with that patient's written permission when necessary to access requested services.

The medical case manager, in partnership with the client, will assist in fulfilling unmet needs. Medical case managers may meet clients at a hospital, medical specialist office or other agencies in an effort to support clients in accessing resources. Clients usually meet with their medical case manager in their office in the clinic.

Medical case managers work closely with the clients to assist in accessing information about community resources that are available to help meet needs. These resources address a number of areas including medical, economic, and basic needs, such as:

  • Drug/alcohol treatment referral
  • Food resources
  • HIV information
  • Housing referrals
  • Legal services
  • Medical equipment
  • Medical insurance
  • Mental health treatment referral
  • Prenatal care
  • Social security benefits
  • Special pharmaceutical benefits
  • Support groups
  • Transportation

Specialized Case Management Services

Eligibility Specialist – assists patients with accessing medical insurance, Social Security benefits, or Pennsylvania's Special Pharmaceutical Benefits Program (SPBP). The Eligibility Specialist also certifies eligible clients for Ryan White services.

Prenatal program – the medical case manager works with pregnant women to reduce the risk of transmission of HIV to the infant. The medical case manager provides support and assistance in accessing resources during pregnancy and attendance at prenatal and post-natal medical visits.

Clients can access medical case management at the Partnership by meeting with the medical case manager on call during clinic visits. If a client needs case management services, they can request to speak with a medical case manager during a medical appointment at the Partnership or call the clinic number, 215.762.2530, and leave a message in the Case Management voice mail box asking to be referred for medical case management services. The client must be a patient at the Partnership clinic in order to receive medical case management services from Partnership medical case managers.
