Dependents Your children or your spouse (wife/husband)
Co-pay Payment required when you go to the doctor or hospital, or when you fill a prescription at the pharmacy.
Deductible Amount you have to pay before the insurance pays
Waived Not required
Reimbursements Money that is returned or repaid to you by the insurance company after they process your claim.
Maternity Pregnancy
Prescription An order from a doctor for medication

Referral A form required to see special doctors
Submitted Given to insurance or doctor
Nurse Practitioner
A nurse who asks questions at the doctor's office or hospital. Nurse practitioners are able to treat basic injuries or illnesses and able to prescribe medicines.

Doctors in the network of your insurance. 90% of the cost is paid by your insurance.
Doctors out of the network of your insurance. 70% of the cost is covered by your insurance.
