A
faculty member who terminates at the end of the spring
term may have his/her unpaid earnings for the current
academic year included in his/her June check if a “Termination
of Faculty Contract” Form (D-57) is signed by the faculty
member and approved by the Provost prior to June 7.
The forms are available in the personnel office. Completion
of this form amends the faculty member’s existing employment
contract by changing the date of termination. This action
affects the termination of the employee’s staff benefits,
which would then be terminated on the new date of termination
(health plan coverage, if the employee is a participant,
terminates as of the end of the month following the
new date of termination.) If s/he so desires, a deposit
will be made at the end of June for the balance of his/her
unpaid earnings for the current academic year to any
bank account that s/he designates, by completing, prior
to June 15th, the proper form available in the records
and data section of the personnel department. For a
terminating faculty member who requests any pay prior
to June 30th, a salary advance in the amount of his/her
regular June check can be arranged.
