Provide transcripts from all colleges and universities attended.
Transcripts must be evaluated by an approved agency for admittance into this program. For more information on transcript evaluations, please refer to the Admission Application Instructions.
GRE test scores are required, minimum score varies.
TOEFL scores are required for international applicants or applicants who earned a degree outside the U.S. (minimum scores: 90/577/233). Scores will be reviewed based on section scores and total scores. IELTS scores may be submitted in lieu of TOEFL scores.
Your essay should address the following points:

Three letters of recommendation are required. Recommendations should include one from a previous supervisor and one from a mentor/advisor.
Use the Electronic Letter of Recommendation form to submit your recommendation letters.
Print and complete the I-20 Application Form [PDF] and submit it with your supplemental materials.
The department will contact competitive applicants who have completed applications for an interview.
Submit all college/university transcripts. Applicants must provide proof that they have earned at least a bachelor's degree (a master's, DPT, or other advanced degree is preferred) from an accredited U.S. college or university, or an equivalent degree from a non-U.S. university or college. A degree in physical therapy is not required. Applicants from all health or rehabilitation-related backgrounds (e.g. kinesiology, occupational therapy, exercise science, biomedical or rehabilitation engineering, etc.) are welcomed.
It is preferred that applicants with clinical degrees (PT, OT, etc.) have two years of relevant clinical experience beyond that required to complete the professional degree.
