Locate last year's Excel sheet containing your chemical inventory. If this is the first time you are filling out the chemical inventory form, click here to download the Excel sheet.
Fill in the form and save it in your My Documents folder.
Click the browse button and select the excel sheet from your My Documents folder.
Click "Submit". This sends a copy of your chemical inventory form to the Department of Safety and Health.
