Bring this form to your thesis defense for committee signatures after successful completion. Once signed by the committee, get signatures of both the MS program director (‘academic adviser’) and the department head. With all signatures, bring this form with the pages of your thesis to the library for binding. (Please follow this link for specifications about paper quality.) Program Completion Form: Use the Graduate College “Completion Form” which can be found under ‘Graduation Forms’ on the Graduate College Website. Psychology signatures needed include mentor (‘supervising professor’) and MS program director (‘department graduate adviser’). Once psychology signatures are obtained, bring this form to the library with your thesis and Thesis Approval Form for library’s signature.
