You must follow the directions below in order to qualify:

SUBMISSION:

• Email your submission to Amanda Keen at amk377@drexel.edu.
• In the body of your email, include your name, address, telephone number (day-time), fax number, email address.
• Please indicate whether you are a nursing educator or practicing nurse.

ATTACHMENTS:

1. Submission form - Please download and fill out the PDF form by clicking on the graphic below:

2. A supporting letter from the Dean or Chair of Nursing if you are an educator or the Chief Nursing Officer of your institution/agency.
3. Two additional letters of support (e.g., from colleagues) stating why the technology is important and innovative.
4. PLEASE NOTE: All three letters of support must be on official institution/agency letterhead and may be scanned for email, or FAXED to 215-762-1259, Attn: Amanda Keen.
5. Your CV.
